INSTRUCTIONS: Please fill out the form completely showing telephone number and the hours you can be reached. After completing this form, mail all copies to SCM HOMES, Attn: Customer Service. NOTE: Request for service will not be acknowledged unless it is reported to our office in writing. Buyer must be available for inspections/repairs. A legal Right of Entry will ONLY be used in the case of an emergency.
